The Darling Downs and West Moreton PHN Health Needs Assessment (HNA) identifies the health priorities and opportunities for our region.
The HNA brings together information from the community, local health services and healthcare providers. Through the HNA, we offer a clear picture of how we can help improve the health of people living in our region. This includes programs and services that can provide people with the right care, in the right place, at the right time.
We have identified ten community-driven priorities for the region through a rigorous process of research and consultation, focused around the seven national PHN priority areas.
We produced the first HNA for the Darling Downs and West Moreton region in 2017. The field of Health is constantly changing to include new evidence-based interventions and models of care, and the needs of a changing population. We therefore regularly update the HNA with new data and analysis, and information gained from engaging with our stakeholders, services and the community.

Joint Regional Needs Assessment
Working with Darling Downs Health and West Moreton Health, we have recently introduced the Joint Regional Needs Assessment (JRNA). The JRNA is a formal way of collating the information and insights from our three separate health needs assessments. It provides a 'whole-of-region' snapshot of the health situation and needs within both primary care and the hospital sector. The JRNA is a recognition of the fact that coordination and collaboration among health providers and services are crucial to provide the best care for patients, especially those most in need.
This initial JRNA identifies the health and service priorities and opportunities for our region from 2025 to 2028.
Community-driven priorities
